Workflow settngs showing multiple versions

As a part of our operations we have a checklist form that flows through approvals during different stages of the project.  A few months back, someone made a change to the base workflow, and ever since we are getting replicated versions of the workflow in the Workflow Settings View.  It was north of 1,600 workflows today.  One of our approvals is on the Project Initiation Phase, just today, several versions were created.

Is there any way to see what even triggered the creation?  We have tried multiple approaches to duplicate the result, but it seems really random.  Is there a log record that would show what created the new versions resulting in an every growing list of "Previous Versions".
