Access 2013 Provisioning Database Fails

We are not able to get Access 2013 working on 2 of our environments. When we add an Access 2013 App to our sites, they eventually fail after about 5 mins. The errors in the ULS logs are as follows and indicate a SQL problem:

The App creation process then detects that the database creation failed and sends back the following entry to the logs

On the SQL side, we can see the db files being created, but SMS becomes unresponsive as the db is attempted to be created then fails to roll back. Site operations to SharePoint are not affected. SQL uses AlwaysOn and SQL 2012 R2 .

Has anybody seen this before?
