Populate 'Links' list in SharePoint via Workflow

I am having difficulty populating a single item on a links list.  The links list is OOB no fields added or removed.  The two fields that are required are the Title and URL.  I can create items manually just fine.  When I try to create a list item through a workflow, it asks for a field called 'Path and Name'.  I am not sure what to put in this field.  I have tried a few things, but it continues to create errors when I run the workflow.  

Does anyone know how to populate this field correctly?
