Environment Tracker for SharePoint servers

Hi Guys,

              This question is intended more for the SharePoint architects, so i thought it best to post it here. I want a generic excel template in order to track all details related to my SharePoint server Farm. For a large farm, this excel sheet can be used to track information like which web application/Site collection/site belongs to which team, is a part of which physical server, who is the application contact for a particular (say) site, etc. I am sure SharePoint architects handling large farms must be aware of such a template.

The template doesn't need to be in excel....what i require is to provide an excel template with column headings, which will be filled up by my client and sent across to me. Analysing this sheet should give me a complete view of the SharePoint farm environment. I need help in getting the column headers.
