Identify a Sharepoint Portal for a Team Project when configuring manually

We had a TFS 2010 deployment configured with SharePoint Server 2010 on SQL 2008 R2 Database and we have regular backups. It so happened that I migrated the TFS Server alone to TFS 2012 (Database to SQL 2012) without moving the SharePoint or Reporting Databases. Later I upgraded the reporting and SharePoint Databases too. SharePoint Server version was still 2010. As for the servers, I have used the same old SharePoint Server as our new SharePoint App-Tier.

TFS Server: Changed Server (Upgraded from TFS 2010 to TFS 2012)

Database Server: Changed the Server (Upgraded from SQL 2008R2 to SQL 2012)

SharePoint Server: Used the Same Old Server. (Used the same SharePoint 2010 to reconfigure)

Now I'm configuring the SharePoint sites manually with the TFS Projects as I was unable to configure them automatically through the TFS console.

In the meantime one of the developers wanted to have access to a SharePoint site configured to a TFS Project "Project XYZ" before it was upgraded. I assumed that the name of the SharePoint site is similar to its respective Team project (TFS). However I couldn't find the SharePoint Site "Project XYZ", probably it had a totally different name, probably the SharePoint portal was created first with a totally different name and configured to Team Project "Project XYZ" manually.

How would I know to which SharePoint site the Team Project XYZ was configured in the first place so that I can configure it to the appropriate portal. The SharePoint site probably has some saved documents too under Shared documents, so configuring a new SharePoint site is not an option.
